Our office is located on the south side of Pine Ridge Road, just east of the Livingston Road intersection, in Cambridge Square. Once you are inside of the building, take the elevator to the second floor and make your way to Suite A. There will be signs to guide you.
There are two ways you can make an appointment:

Should you choose to fill out our online form, one of our dedicated appointment schedulers will contact you within 48 hours to confirm the date and time of your appointment.
Yes! At the Joint Replacement Institute, we want to ensure each patient feels comfortable and confident in their care. For this reason, we encourage our patients to select the provider they wish to visit during their visit with us.
If you are not yet familiar with our providers, you may also choose to be seen by whichever provider’s availability best suits your schedule and individual needs. Rest assured, each of our physicians and highly skilled, qualified, and ready for your care.
When preparing for your visit, we kindly ask that you bring:
- Photo ID
- Insurance card
- Completed patient forms
- Relevant imaging and medical records (if applicable)
- Physician referral paperwork (if applicable)

If you need to cancel your appointment, please call our office at (239) 261-2663 as soon as you can. We kindly ask that you provide at least 24 hours' notice prior to your scheduled appointment time, so that we may accommodate other patients.
If you would like to reschedule your canceled appointment, our friendly staff will assist you in finding a convenient new time.
Yes, Joint Replacement Institute accepts workers’ compensation, Medicare, and most major insurance plans. To confirm if we accept your specific insurance, please contact your insurance provider or call our office at (239) 261-2663.
If your plan includes co-payments, co-insurance, or a deductible, these must be paid at the time of your visit. For your convenience, we accept payments via cash, checks, Visa, MasterCard, Discover, and American Express.
You may need a referral to make an appointment at the Joint Replacement Institute, depending on your insurance plan. If your insurance is an HMO or managed care plan requiring a referral from your primary care physician, ensure it is arranged before your orthopedic visit.
For physical therapy appointments, a referral from your medical provider is also necessary for insurance coverage. Referrals can be submitted by your provider via fax or phone, or you can bring a signed referral form to the office. Appointments may need to be rescheduled if required referrals are not received in advance.
